All organizations who offer health insurance, and other health and welfare benefits are required to provide employees with a summary plan description for each benefit. Under this service, we create one summary plan description that includes all health and welfare benefits, along with required ERISA language.
Organizations with 100 or more plan participants at the beginning of the year are required to complete form 5500 reporting on their health and welfare benefits. We will complete this report for you each year

The following Terms and Conditions apply to and are incorporated as a material portion of the Agreement between the Member ("Client"), and HR Service, Inc. ("HR Service").
Use of Intellectual Property
Client agrees to use supplied updates, materials, templates, handbooks, forms, and other provided materials or intellectual property only for their organization and will not allow access to such materials by any outside party. HR Service shall be the sole owner of the copyright concerning all materials which it shall provide pursuant to this agreement.
HR Service Acknowledgement/Waiver
Client acknowledges that the services rendered by HR Service pursuant to this agreement are not legal service or legal advice, but are services similar to those rendered by a company’s non-lawyer human resources personnel. HR Service will use its best efforts to assist Client in complying with employment laws and regulations. However, Client is and remains ultimately responsible for complying with all applicable laws and regulations. To this end, Client agrees to indemnify HR Service, its directors, officers, employees, and agents from all claims, expenses, and losses of every kind arising from any updates, advice, coaching or information received from HR Service.
Terms
Client agrees to pay HR Service, Inc. for any selected services not already paid by another party (e.g. broker). All billing cycles are on a monthly or annual basis with fees being automatically drafted or charged to a credit card or bank account. Monthly invoicing is available for purchases in excess of $250 upon request.
